Key Responsibilities
? Conduct comprehensive in-home evaluations to assess patient needs and develop tailored care plans.
? Collaborate with Physical Therapy Assistants (PTAs) to execute care plans effectively.
? Facilitate improvements in patient mobility and independence through targeted rehabilitation strategies.
? Maintain open communication with patients' physicians to ensure continuity of care and optimal outcomes.
? Evaluate and document patient progress, providing updates to medical professionals as necessary.
? Supervise a team of 2 PTAs, ensuring adherence to performance standards and best practices.
Required Skills
? Current, unrestricted California Physical Therapist License.
? Minimum of 1 year of experience supervising PTAs in a home health setting (preferred).
? Valid BLS/CPR Certification and California Driver?s License.
? Knowledge of local community resources and their relevance to patient care.
? Strong understanding of individual development, human behavior, and cultural influences in healthcare.
? Familiarity with state and federal homecare regulations (preferred).
? Experience working with high-acuity patients and interdisciplinary teams (highly desired).
? Proficiency in case management fundamentals (highly desired).
? Exceptional organizational, interpersonal, and leadership skills.
Qualifications
? Self-direction, empathy, and solid critical thinking capabilities are essential for this role.
? A post-secondary degree in Physical Therapy or an equivalent qualification is required.
