About the position Responsibilities • Assists team leader and/or optical vendor scheduling schools for mandated vision services. • Assists school and optical vendor escorting students to designated vision testing area. • Assists team leader and/or optical vendor creating a conducive testing environment. • Act as liaison for Program in absence of Team Leader and reports any issues to Regional Supervisor. • Import class lists/biographical information to Spot autorefractor device from ASHR. • Provide vision screenings, using the Spot device. • Screen students using a Snellen eye chart for distance vision, hyperopia +2.50 lenses, near vision test, color and fusion tests. • Assist students selecting proper eyeglass frames. • Refer students for any possible vision problems. • Set up vision screening equipment at beginning of each day and securing vision screening equipment when screening is completed. • Complete referral forms issued to parents of students who fail screenings. • Assists with follow-up in obtaining updated phone numbers from schools for students who fail screenings, providing data daily to Follow-up Unit. • Maintain and transport vision screening equipment and supplies to and from schools. • Communicating to students and staff in a courteous/professional manner and assist screening team leader as directed. • Recording, tallying, and entering screening data accurately on student's worksheets, Teacher's yellow health cards (104s), screening log and ASHR (OSH automated student health record). • Export screening data from Spot device to ASHR. • Other duties as assigned by Program. • City Wide travel and flexibility with location of assignment may be required based on programmatic needs. Requirements • A baccalaureate degree from an accredited college or university, including or supplemented by twelve semester credits in health education or in health, social or biological sciences. • A baccalaureate degree from an accredited college or university, and six months of full-time satisfactory experience in a health promotion or disease intervention/prevention program. • A four-year high school diploma or its educational equivalent approved by a State's Department of Education or a recognized accrediting organization and four years of full-time satisfactory experience in health promotion or disease intervention/prevention. • Education and/or experience equivalent to the above. Nice-to-haves • Experience as a Public Health Adviser (School Health), Assignment Level I. • Experience in conducting field investigations, assessing health risks, making referrals, or collecting and analyzing epidemiological data.
